This policy explains what personal information Bella Viságe Medical & Aesthetic Rejuvenation
collects when you use bellavisagelakeland.com, why we
collect it, who else can receive it, and the choices you have. It also lists every cookie and tracking
technology on this website, and how to switch them on or off at any time.
We have written this in plain English on purpose. If anything here is unclear, email
admin@bellavisage.info or call
863.333.0553 and we will explain it.
This policy covers this website. It does not cover the treatment records we keep about patients in
our practice, or the separate systems we use to run appointments in the clinic.
Part 1 — Privacy
1. Information we collect
Information you give us. When you complete a consultation request, an inquiry form
or an offer form on this site, we collect what you type into it. That is normally your name, email
address and phone number, and anything you choose to write in a message box, which may include the
treatment you are interested in.
Please do not send us detailed medical information through this website. The forms
on this site are for arranging a conversation, not for taking a medical history. If you need to
discuss something private, call us or raise it at your consultation.
Information collected automatically. Like almost every website, our web server
records the IP address that requested each page, the date and time, the page requested, and the
browser and operating system reported by your device.
Information collected by the tracking tools listed in Part 2. These only run if you
agree to them. They are described in full below, including one that records how pages are used.
2. Why we use it
- To answer your inquiry and arrange a consultation or appointment.
- To send you information you asked us for, and offers if you asked to receive them.
- To keep the website working, secure and free of spam.
- If you agree to it, to understand how the website is used and to measure our advertising.
3. Consent, and how we handle tracking
When you first arrive, a banner appears at the bottom of the page. Until you make a choice there,
the statistics and marketing tools listed in Part 2 do not run. Their code is
delivered to your browser in a switched-off state and is only switched on if you agree.
Only the strictly necessary items in Part 2 load before you choose. Those are the things the site
needs in order to work at all, and they cannot be switched off.
You can change your mind at any time using the Manage cookies button in the bottom
left corner of every page.
4. Who else can receive it
We do not sell your personal information for money.
Your information is handled by companies that provide services to us, and by the tracking companies
listed in Part 2 if you agree to them. In plain terms, the categories are:
- The company that hosts this website and stores its data.
- The company that runs our inquiry forms and our email and text follow up.
- Our appointment booking system, when you click through to book.
- Our email provider, when we reply to you.
- If you agree to statistics or marketing cookies, the analytics and advertising companies named in
Part 2. Some of those companies use what they collect for their own advertising purposes as well as
ours. - Professional advisers, and anyone we are legally required to give information to.
5. How long we keep it
Inquiries that do not become appointments are kept for 24 months and then deleted. Records relating
to patients and to payments are kept for as long as the law and our professional obligations require.
Records of cookie consent are kept for at least 24 months, or for the duration of your treatment with us if you become a patient, so we can show what was agreed and when.
6. Your choices and your rights
- Cookies and tracking. Use the Manage cookies button at any time, on any page.
- Marketing. Every marketing email has an unsubscribe link. For texts, reply STOP.
- Access, correction and deletion. You can ask us what personal information we hold
about you, ask us to correct it, or ask us to delete it. Email
admin@bellavisage.info. We will respond
within 45 days. There are some records we are required to keep even if you ask us to delete them, and
we will tell you if that applies. - Browser controls. Your browser can also block or delete cookies. Blocking
everything may stop parts of the site working.
7. How we protect it
This website is served over an encrypted connection. Access to the systems holding inquiry data is
limited to people who need it. No website can promise perfect security, and we do not claim to.
8. Children
This website is intended for adults. We do not knowingly collect personal information through this
website from anyone under 18. If you believe a child has sent us information through the site, contact
us and we will delete it.
9. Other websites
This site links to other websites, including our booking system and our social media pages. Once you
follow a link, you are on someone else’s website and this policy no longer applies. We are not
responsible for their content or their privacy practices.
10. Changes to this policy
If we change this policy we will update the date at the top. If we change it in a way that affects
how tracking works, the consent banner will ask you again.

Part 2 — Cookies and similar technologies
What these are
Cookies are small files a website stores in your browser. Similar technologies do the same job by
other means, including code that runs in the page and small invisible images used to count visits. In
this policy, “cookies” means all of them.
The three categories
The banner divides everything on this site into three groups. The first is always on. The other two
are off until you agree.
Strictly necessary — always on
| What | Provided by | What it does |
|---|---|---|
| Website session and security | This website | Keeps the site working, and remembers your cookie choice so the banner does not ask again. |
| Inquiry and consultation forms | HighLevel (LeadConnector) | Runs the inquiry and offer forms on this site and passes what you submit to us. It stores what you have typed in your own browser so a part completed form is not lost. |
| Contact forms | Gravity Forms, on this website | Runs contact forms and guards them against spam. |
| Accessibility toolbar | MaximizeADA | Provides the accessibility controls, and remembers the settings a visitor chooses. This is assistive technology, so it is not switched off by the cookie banner. |
| Images and page files | Our content delivery network | Serves our own images and files quickly. It does not track you across other websites. |
Booking. When you book, you click through to our booking system, Zenoti, on its own
website. That is a link, not something running inside this page, and Zenoti’s own privacy terms apply
once you are there.
Statistics — off until you agree
| What | Provided by | What it does |
|---|---|---|
| Google Analytics | Counts visits and shows which pages are used. It receives your IP address. | |
| Google Tag Manager | A container that loads the other Google tools listed here and in the marketing table. | |
| Microsoft Clarity — session recording | Microsoft | Records how pages on this site are used, including mouse movement, scrolling and clicks, and replays it to us as a recording. We use it to find parts of the site people struggle with. It does not run unless you agree to statistics cookies. |
Marketing — off until you agree
| What | Provided by | What it does |
|---|---|---|
| Meta pixel | Meta (Facebook and Instagram) | Measures our Facebook and Instagram advertising and can be used to show you our ads on those platforms. |
| Google Ads conversion tracking | Measures which adverts led to an inquiry, and can be used to show you our ads elsewhere. | |
| MNTN | MNTN | Measures our connected TV and streaming advertising, including whether a visit followed an advert. |
| Embedded video and audio | YouTube, Simplecast | Videos and podcast players embedded in some pages. Until you agree, these show a placeholder with a button to load them, so nothing is requested from those companies unless you ask for it. |
Changing your choices
Use the Manage cookies button in the bottom left corner of any page. You can accept
everything, reject everything, or switch statistics and marketing on and off separately. Rejecting
does not affect your ability to browse the site, send us an inquiry or book an appointment.
Do Not Track and Global Privacy Control
Because nothing in the statistics or marketing categories runs until you actively agree, this site
does not track visitors who have expressed no preference. If your browser sends a Global Privacy
Control or Do Not Track signal and you have not made a choice here, nothing in those categories
runs.
