Terms & Conditions
- Membership Fee: $99/month, charged via auto-payment from the card on file. NO INITIAL FEE.
- Billing Cycle: Auto-renews each month on the 1st or 15th, whichever date is soonest at the time of sign-up. Auto-renews each month unless terminated at least two (2) weeks before the next renewal date.
- Payment Details & Late Payments: In the event of a failed payment, the membership benefits will be paused until the payment is resolved.
Cancellation Policy
- Cancellation Notice: Members may cancel at any time with two weeks’ written notice.
- Cancellation Request Submission:
- All cancellation requests must be emailed to admin@bellavisage.info at least two (2) weeks prior to the next scheduled payment.
- Requests received after this period will take effect in the next billing cycle, with no refunds for the current month.
Membership Renewal
- Memberships automatically renew each month unless a cancellation is submitted in accordance with the policy above.
- Memberships are non-transferrable and intended for use by named member only.
Bella Neurotoxin Membership Agreement
By filling out this form and signing this agreement, I confirm that I have read, understand, and agree to the terms and conditions of the Bella Neurotoxin Membership. I authorize Bella Viságe Medical & Aesthetic Rejuvenation to charge my credit card on file for the monthly membership fee of $99 until I cancel according to the terms outlined above.
